- Issue identification number
- Issue name or title
- Date the issue was submitted to the project team
- Individual who submitted the issue to the project team
- Individual who captured the issue information
- Project phase potentially impacted by the issue
- Issue type, description, priority, and category
- Level of authority required to address the issue
